A 27-year-old nurse’s aide who recently moved from another state and reports having a rare genetic disorder of platelet function presents to the hospital reporting recent nosebleeds and uncontrolled menstrual bleeding. The answer is D. A recent systematic review of 455 cases of factitious disorder (the largest sample analyzed to date) found that 59% of patients chose to self-induce illness or injury rather than attempting only to simulate (19%) or falsely report (22%) a medical problems. This suggests that what has previously been regarded as an extreme variant of factitious disorder may in fact be its most common presentation. This underscores the potential safety risk and importance of continuous observation for these patients while hospitalized.
