A 20 year old man is brought to the hospital for strange behavior. His grandmother reports that he punched his younger brother, and prior to that had spend 3 months mostly in his room. He has never been violent before. Prior to that, she reports he seemed “completely fine.” While he appears to get angry sometimes, most of the time he seems “disconnected” and “numb.” He sleeps 8 hours a night and eats well; she reports that he appears to be writing in journals when he is alone in his room. On interview, he refuses to verbally tell you why he punched his brother “because someone is listening” but writes down on a piece of paper “he and my grandmother have become possessed by Satan.” On mental status exam, his affect is blunted and he reports his mood as “fine.” His thought process is linear and he has thoughts that people around him are possessed. He denies auditory and visual hallucinations. He denies suicidal or homicidal ideation. Urine drug screen is negative. What is the most likely diagnosis?
