You are consulted for management of delirium in a 53- year-old woman with metastatic renal cell carcinoma, severe pain, and metabolic disarray. Her pain is being managed unsuccessfully with a morphine-based regimen. Part of the problem is that she seems to become increasingly confused with the morphine even though it is underdosed.
